How To Edit Your Template and Craft Effective Context Instructions

Step-by-Step Instructions On How To Customize The Format Of Your Notes

Start by watching our YouTube tutorial on customizing your patient notes.

How to Customize The Format of Your Patient Notes:

1. Duplicate:

  • First, duplicate the template you would like to customize.

  • To do this, access your templates in the Left Side Bar.

  • Right-click the Template and select 'Duplicate' to make a copy.

2. Navigate To The Flow Editor:

  • In the top right beside the blue Share button, click the Edit button to open the Flow Editor.

  • The Flow Editor is where you unlock the magic of Lindy's customization features!

3. Click the Settings of your Template.

4. Give Your New Template a Name:

  • Provide a name for your scribe, making it easy to identify.

4. Review and Edit the Scribe's Context:

  • You will see a section called Context.

    • The context contains a set of instructions that your medical scribe uses to structure your patient notes.

    • Now that you have made a copy, you can edit them to suit your charting preferences! Let's get started 😊

* We recommend you review the current intructions before making edits *

  • After reviewing, you may delete, edit, or add instructions in the Context field.

  • You can edit as much or as little as you prefer to achieve your desired patient note format.

  • Specify sections of your patient note that you want to copy and paste by beginning the section with:

    • # for the Main Header

    • ## for Header 2

    • ### for Header 3

To help emphasize important aspects of your instructions, you can use asterisks (*) around text which signifies they are BOLDED.


💡 Tips for Creating Effective Context Instructions:

Remember, Lindy can format the patient notes in any format you'd like!

Think of the Template Context as a set of instructions that you would give a medical assistant. The medical assistant will receive your patient visit transcript, and follow the Context you specify, to deliver you the best patient note possible.

Start Your Instructions by Giving Specific Context

  • Specialty and Context Awareness: Specify your specialty at the beginning of your Context Instructions. You can also include context about the type of patients you see, or the nature of the visit.

    • For example: "The user is a Psychiatrist. The patient visit will be a first-time consultation"

Be Explicit With How You Want The Note Structured

  • Be Direct: State exactly what you want. Skip pleasantries — instead, specify exactly how you want Lindy to format the patient note.

    • For example: "You must document this section in paragraph form, use direct quotes from the patient and never include medication in the HPI"

  • Avoid Contradiction: Ensure you remove any conflicting instructions.

    • For example, if you do not want a standard SOAP note(Subjective, Objective, Assessment and Plan) → remove all language and instructions regarding a “SOAP note” and switch to your desired format

    • If you want the note in paragraph form (NOT a bullet point form list), remove any instructions requesting bullet point form.

  • Action-Focused: Use active language. Write “do” over “don't”

    • For example: "Always document the Plan in bullet point form and outline these subsections: Aftercare, Discharge Information, Follow-Up Information, Patient Education"

  • Step-by-Step: Break down the instructions for how you want the patient note structured into steps, clearly guiding Lindy through each part of the process.

    • For example, "First, you must give a summary of the patient update, then document a through HPI, then suggest ICD-10 Billing Codes"

  • Keywords and Phrases: Incorporate essential words to emphasize important aspects of your directions:

    • “Your task is”

    • “You MUST”

    • “You need to”

  • Clear Formatting: Separate instructions with line breaks for clarity.

    • Using "— — " can act as a line break.

Customize the Sections You Want To Copy

  • Specify sections of your patient note that you want to copy and paste by beginning the section with:

    • # for the Main Header (Header 1)

    • ## for Header 2 (Think of this as a subsection)

    • ### for Header 3

  • You can customize as many Headers / Sections as you would like within your patient note. The 📑 icon beside the section header is where you can click to copy the desired section.

Tailor Your Scribe To Chart Like You Using Examples

  • Provide Examples:

    • Adding Examples to your context instructions will help improve quality and reliability. Lindy learns very well by mimicking given examples.

    • If there is a section of the note you want Lindy to document in a specific way, provide an example (or two) for how you would like it to read.

    • To provide an example, use this text (replace the quoted example):

      Here is a perfect example of how the user would like the [Insert section / subsection]:

      "Subjective:

      The patient, Ashley, reports no significant improvement or changes in symptoms since starting thyroid medication, which was prescribed by a hormone-based practice. She has been taking NP Thyroid, a 30 mg tablet daily, for at least a year. Staci has not experienced any changes in hair quality, exhaustion, or diet-related issues."

      Mirror the level of detail, langugae, tone and structure when documenting this section of the note. Only use the transcript for context.

  • Teach Your Scribe To Chart Like You:

    • Adding 1 - 3 examples is the best way to have Lindy chart like you.

    • Try adding this to the end of your Context Instructions (replace the quoted example):

"Subjective:

The patient, Ashley, reports no significant improvement or changes in symptoms since starting thyroid medication, which was prescribed by a hormone-based practice. Test Your Templates

  • To test your templates, try pasting a previous patient visit transcript into the chat, press send, and see how the patient note changes based on your Context Instructions.


Do's for Lindy Formatting Context Instructions:

  • Proofread: Review your formatting Context Instructions for any errors or ambiguities before finalizing them.

  • Practice Patience: Lindy may require multiple attempts to achieve the desired format for patient notes. Be patient and iterate as needed. (Let us know if you need help!)

  • Learn from Feedback: Pay attention to the results of Lindy's formatted patient notes and adjust your Context Instructions based on the outcomes.

  • Don't Overcomplicate: Keep your formatting Context Instructions straightforward and easy to follow for Lindy.

  • Avoid Repetition: Refrain from submitting the same formatting Context Instructions repeatedly if you're not achieving the desired format for patient notes.
